I.T.A.S.C.A.R. was created with a clear aim: to build a shared, objective, and reproducible language for the evaluation of acne scars..

Developed by the clinical dermatology team of Dermacademy, I.T.A.S.C.A.R. is a scale designed by clinicians for clinicians, to overcome subjectivity in the evaluation of acne scars and to make therapeutic outcomes measurable and comparable, both in daily clinical practice and in research.

Why I.T.A.S.C.A.R.

In the treatment of acne scars, the lack of a simple and standardized metric has long represented a clinical and scientific limitation.

I.T.A.S.C.A.R. was designed to be:

  • Simple – usable within a few minutes during the consultation
  • Rapid – applicable in routine outpatient practice
  • Reproducible – independent of the operator
  • Sensitive to change – suitable for follow-up and for comparison between treatments

The system is based on two fundamental principles:

  • Standardized facial areas, defined by constant anatomical landmarks
  • Weighted scoring, which reflects true clinical impact by combining the number, size, and severity of scars

How I.T.A.S.C.A.R. works

Facial subdivision

The face is divided into four standardized anatomical areas:

  • Forehead
  • Right cheek
  • Left cheek
  • Central area

This subdivision is based on stable anatomical landmarks (ocular canthi, oral commissure, mandible), ensuring that each clinician assesses the same areas, in the same way.

Assessment of each area

For each area, three scores are assigned:

  • Score A – Total number of scars
  • Score B – Number of scars > 2 mm
  • Score C – Clinical severity, also assessed using the skin “stretching” maneuver

Score calculation

  • Area score:: A × B × C
  • Total facial score: sum of the four areas
